County Health Department COIVD-19 Vaccine Survey Online

The Madison County Health Department now has a survey on their website for residents interested in receiving the COVID-19 vaccine.

Residents who complete the survey will be contacted by the health department when the vaccine is available and appropriate based on Illinois Department of Public Health guidelines.

In accordance with United States Center For Disease Control guidance, the COVID-19 vaccine will be distributed in phases to different priority populations. Each phase will take some time to distribute.

The Madison County Health Department will alert the public as to the phase, the priority population for that phase, and where, when and how to get the vaccine.

Stay tuned to local media, Madison County Health Department social media sites, and the website www.madisonchd.org for information as it becomes available.

    My name is Holly, and I am a 21 year old female. My mother, Jana, is a nurse at Barnes Jewish Hospital. She has received the vaccine. I have Hashimotos, a thyroid condition, which is considered an autoimmune disorder. I understand that there is a much higher need for this vaccine in the elderly community; however, I am very interested in receiving the COVID-19 vaccine.
